What should you know about early stages of bladder cancer?

Bladder cancer often begins in the inner lining. Non-muscle-invasive disease has not grown into the bladder muscle; muscle-invasive disease has reached that deeper layer but may still be localised. Cystoscopy lets a clinician inspect the bladder and remove tissue for examination. Scans and laboratory findings add information about nearby structures.

Blood in the urine, changes in urination or difficulty passing urine can have several causes. Difficulty passing urine with severe lower abdominal discomfort may require urgent assessment because retention can strain the urinary system.

Treatment depends on depth, tumour features, number of areas involved and previous findings. A procedure through the urinary passage may remove visible disease. Intravesical BCG Therapy places medicine inside the bladder to lower the chance of recurrence for selected non-muscle-invasive cancers. More advanced local disease may lead to discussion of Bladder Removal Surgery, chemotherapy or immunotherapy.

Keep follow-up cystoscopy appointments even when you feel well. Ask how to manage urinary symptoms, smoking cessation and fluid advice around procedures.

Seek urgent medical help for inability to pass urine, heavy or persistent blood in the urine, clots that block flow, fever with urinary symptoms, severe pain, or faintness. After a procedure, use the care team’s emergency instructions for worsening bleeding or fever.

Ask: Has the cancer reached muscle? What did the tissue sample show? How often will cystoscopy be needed? Is intravesical treatment suitable? Could I need bladder removal, chemotherapy or immunotherapy? What symptoms should trigger a same-day call?
