Ear infection without a fever: what to know

Fever is only one possible sign of infection. Ear inflammation may mainly cause pain, pressure, reduced hearing, itch, or fluid from the ear. Adults and children can have different patterns, and pain may come from the ear canal, middle ear, jaw, teeth, or nearby throat. A healthcare professional can use an otoscope to look for swelling, fluid, or changes to the eardrum rather than relying on temperature alone.

Note when the pain began, whether hearing has changed, and whether fluid is coming out. Keep the ear dry and avoid inserting buds, fingers, or home remedies. Use age-appropriate pain relief only as directed on the label, taking account of allergies and other medicines. Book an appointment if pain does not ease, keeps returning, or follows swimming, a cold, or an injury.

Seek prompt medical care for discharge with severe pain, sudden hearing loss, swelling behind the ear, facial weakness, severe dizziness, or a child who is unusually sleepy or difficult to settle. Emergency help is needed if the person is confused, has a severe headache with vomiting, or looks seriously unwell, even if their temperature is normal.
