The short answer
A seizure with an ear infection is not a routine symptom and needs emergency medical assessment. Ear pain may occur during an illness, but a seizure can signal fever, a serious infection, a metabolic problem, or another neurological cause that cannot be diagnosed at home.
Why the combination matters
Middle or outer ear infections do not usually explain a seizure by themselves. A high fever may trigger a seizure in some children, while infection spreading beyond the ear can involve tissues around the brain. [Meningitis](/conditions/meningitis/) may cause headache, stiff neck, vomiting, light sensitivity, drowsiness, or a rash. Low blood sugar, withdrawal related to [Alcohol Use Disorder](/conditions/alcohol-use-disorder/), and [Hypoglycaemia](/conditions/hypoglycaemia/) are other possible causes.
During a seizure
Protect the person from nearby objects, cushion the head, and place them on their side when possible. Loosen tight clothing around the neck, watch the movements, and note how long the episode lasts. Do not restrain them, put anything in their mouth, or give food, drink, or medicine until they are fully alert. Stay with them and arrange emergency help.
Emergency warning signs
Call emergency services for a first seizure, a seizure that continues, repeated seizures without recovery, breathing difficulty, injury, pregnancy, or diabetes. Immediate help is also essential with fever and stiff neck, severe headache, confusion, unusual sleepiness, or a spreading rash. Do not drive the person yourself if they remain confused or unwell.
Questions for your doctor
Ask what may have triggered the event, whether blood tests or brain and ear examinations are needed, and how to reduce injury risk if it happens again. Ask which fever, ear, or neurological symptoms require immediate return to emergency care.
