Dyspareunia symptoms: what pain during sex can feel like

Entrance pain may feel like burning, stinging, cutting, rawness, or tightness. It can occur with initial penetration, a tampon, or contact around the vulva. Deeper pain may feel like aching, pressure, cramping, or a sharp pain with deeper penetration or particular positions. Some people notice soreness that continues after sex.

Other symptoms can provide clues without proving a diagnosis. Dryness or itching may point toward tissue irritation or Genitourinary Syndrome of Menopause. Vulval colour or texture changes can occur with Lichen Sclerosus. Pelvic pain around periods can accompany Endometriosis or an Ovarian Endometrioma, while discharge, fever, or bleeding needs assessment for infection such as Pelvic Inflammatory Disease. Vulvodynia is another possible contributor to persistent vulval pain.

Note where the pain is, when it starts, how long it lasts, which activities trigger it, and whether it follows your menstrual cycle. Record dryness, itching, discharge, urinary symptoms, bleeding, or pain outside sexual activity. During sex, use lubricant, slow the pace, and choose activities or positions that do not cause pain.

Arrange an appointment if symptoms recur or change. A clinician may examine the vulva and vagina, assess pelvic-floor muscles, and offer tests guided by your symptoms. Do not use scented washes or self-treat a suspected infection without advice.

Get urgent help for severe sudden pelvic pain, fainting, heavy or unusual bleeding, fever, vomiting, or feeling very unwell. Seek prompt care for new sores, foul-smelling discharge, or pain during pregnancy. These warning signs require assessment regardless of how often pain during sex occurs.

Is my pain superficial, deep, or both? Could dryness, skin change, muscle tension, infection, or endometriosis fit this pattern? Do I need an examination, swabs, imaging, or pelvic-floor referral?
