Dry, flaky spot on the scalp: possible causes

A small area that appeared after a new dye, shampoo, or styling product may be contact irritation. Greasy scale around the hairline or behind the ears can accompany seborrheic dermatitis. A well-defined, thick, silvery patch may occur with scalp psoriasis. Ringworm may produce scale with broken hairs, tenderness, swelling, or patchy hair loss. Appearance alone cannot confirm the cause.

Stop recently introduced scalp products and cleanse gently without scraping the scale. Keep nails short and avoid picking. Do not apply steroid, antifungal, or antibiotic products to the spot without suitable advice, because the wrong treatment can obscure the diagnosis. Avoid sharing brushes, hats, and towels until infection has been considered. A clinician or pharmacist can discuss medicated shampoo treatment when appropriate.

Book an assessment if the patch enlarges, becomes painful, drains, bleeds, forms a thick crust, or causes broken hairs or hair loss. Seek prompt care for swelling, rapidly spreading redness, or marked tenderness. A persistent isolated spot also merits review when simple product changes do not help.

Ask what features distinguish dandruff, seborrheic dermatitis, psoriasis, and ringworm; whether the spot needs a skin or hair examination; and which products are safe near the eyes and ears.
