Dry cough and trouble breathing: when to act

Breathlessness with a dry cough may occur with asthma, a viral infection, acute bronchitis, an allergic reaction, or another heart or lung problem. Wheezing, chest tightness, rapid breathing, or difficulty speaking in full sentences can signal substantial airway narrowing. Sudden symptoms after food, medicine, or an insect sting raise concern for a serious allergy.

The symptom combination cannot be safely diagnosed from the cough alone. A healthcare professional may check breathing effort, oxygen levels, chest sounds, and the timing of the problem.

Stop exertion and sit upright in a comfortable position. Move away from smoke, dust, or a suspected allergen. If you have a prescribed rescue inhaler or allergy action plan, follow it exactly. Do not drive yourself if breathing is severely impaired, and do not wait for a cough medicine to work when air hunger is increasing.

Call emergency services now for severe or rapidly worsening breathlessness, blue or grey lips, inability to speak normally, confusion, collapse, or swelling of the lips, tongue, or throat. Urgent assessment is also needed for new breathlessness with chest pressure, faintness, coughing blood, or a serious allergic exposure. Even milder new breathing difficulty deserves prompt clinical advice, especially in a child or someone with known heart or lung disease.

Ask whether asthma, infection, allergy, reflux, or a medicine could be involved; whether you need breathing tests; how to use prescribed treatment; and which symptoms should trigger emergency action.
