Dry cough with blood in phlegm: what to know

A dry cough may still bring up small amounts of mucus. Pink or red streaks can occur when coughing irritates small surface vessels in the throat or breathing tubes. A common cold, acute bronchitis, allergic rhinitis with drainage, asthma, acid reflux, or chronic cough may present with cough and mucus. Bronchiectasis can also present with recurrent phlegm and bleeding; a symptom alone cannot determine the diagnosis.

Write down how often it happens and whether the material is frothy, thick, or clearly blood. Note fever, chest discomfort, breathlessness, night sweats, weight change, recent infection, smoking, and blood-thinning medicines.

Keep hydrated, avoid smoking and vaping, and do not suppress a productive cough with leftover medicine without advice. If possible, take a photograph for the clinician rather than saving tissue. Book an appointment for any recurrence, an ongoing cough, or blood without an obvious irritated-throat explanation.

Call emergency services for difficulty breathing, severe chest pain, fainting, confusion, blue lips, or a large or continuing amount of blood. Seek same-day medical care for repeated episodes, worsening fever, marked weakness, or new breathlessness even when the amount seems small.
