Dry cough with a barking sound: what should you know?

A barking quality can come from vibration and swelling around the upper airway, or from forceful irritation lower down. The sound alone cannot identify the cause. A recent Common Cold or Acute Bronchitis may leave the airways inflamed. Allergic Rhinitis can cause drainage and throat irritation, while Asthma may produce cough with wheeze or chest tightness.

In infants and young children, Bronchiolitis can cause cough, noisy breathing and feeding difficulty. Acid Reflux can irritate the throat and contribute to coughing, especially after meals or when lying down. Bronchiectasis and Chronic Cough require assessment when symptoms persist or recur.

Keep the person calm and upright, offer fluids if swallowing is comfortable, and move away from smoke, sprays and strong scents. Listen for noisy breathing when resting and note whether the sound began suddenly or followed a cold, exposure or meal.

Do not use someone else’s prescription medicine or force food and drink if breathing or swallowing is difficult. A clinician can decide whether an inhaler, other treatment or an urgent examination is needed.

Seek emergency help for struggling to breathe, a harsh noise while breathing in, blue or grey lips, drooling, inability to swallow, marked drowsiness, or rapidly worsening symptoms. Children with noisy breathing at rest, poor feeding or fewer wet nappies need prompt assessment. Arrange review for a barking cough that persists, returns, or is accompanied by fever, wheeze or chest pain.

Ask whether the sound is coming from the throat or chest, whether infection, allergy, reflux or asthma is more consistent with the pattern, and what signs should trigger urgent care.
