Dry cough and breathlessness: what should you know?

Asthma, acute bronchitis, COPD and bronchiectasis can cause cough with difficult breathing through airway inflammation, narrowing or mucus changes. Breathlessness may also occur with anaemia, angina or atrial fibrillation because the body or heart cannot meet demand comfortably. Anaphylaxis can cause sudden breathing difficulty with swelling, hives or throat tightness.

The pattern matters: note whether symptoms began suddenly, occur at rest or exertion, and come with chest discomfort, palpitations, fever, leg swelling or an allergic exposure. Breathlessness is not a diagnosis, and a cough does not identify which condition is responsible.

Stop exertion, sit upright and loosen tight clothing. Move away from smoke or a suspected allergen. If you have a prescribed asthma or COPD action plan, follow it. Do not borrow inhalers or delay assessment while trying several cough products.

Arrange prompt medical review even if symptoms are mild but new, recurring or limiting normal activity. A clinician may check oxygen level, heart rhythm, lungs and blood count, then choose further tests based on the findings.

Call emergency services for severe or rapidly worsening breathlessness, inability to speak comfortably, blue or grey lips, fainting, confusion, severe chest pressure, or sudden swelling of the lips, tongue or throat. Urgent assessment is also needed for coughing blood or breathlessness that starts suddenly.

Ask whether the symptoms suggest an airway, heart, blood or allergic cause, and which checks you need. Tell the clinician about smoking, previous lung disease, palpitations, heavy periods, new medicines, allergies and the exact timing of the breathlessness.
