Does uterine fibroids cause painful periods?

Fibroids can make the uterus stretch, change its shape or contract more strongly during menstruation. A fibroid may also press on nearby organs or cause a feeling of fullness in the pelvis. Pain may be cramp-like during bleeding, or may include lower back ache, pelvic pressure or discomfort with sex. The location and size of a fibroid influence the pattern, and some fibroids cause little or no pain.

Endometriosis, adenomyosis, pelvic infection, ovarian conditions and other problems can also cause painful periods. A new or worsening pattern should be assessed rather than attributed to a known fibroid without review. Your clinician may ask about cycle timing, pain severity, bowel or bladder symptoms and whether pain occurs outside menstruation.

Arrange an appointment if pain limits work, sleep, movement or intimacy, or if usual pain relief no longer helps. Record when pain starts, where it is felt, how long it lasts and any heavy bleeding, pressure, fever or bowel and bladder changes. A clinician may recommend an examination, ultrasound or other imaging. Discuss anti-inflammatory medicines when suitable, hormonal treatment and procedures, including how each option fits pregnancy plans.

Seek urgent care for sudden severe pelvic pain, pain with faintness, fever, repeated vomiting, heavy bleeding or a possible pregnancy. Prompt review is needed when pain begins between periods, follows sex, steadily worsens or occurs with unusual discharge or difficulty passing urine. These features may point to a cause that needs treatment beyond fibroid symptom control.
