Does patellar tendinitis cause kneecap pain?

Short answer
Kneecap Pain is a common presentation of Patellar Tendinitis. The discomfort is often felt at the lower edge of the kneecap and increases when the tendon is loaded.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Pain location
- Often at the lower border of the kneecap or along the tendon
- Often aggravated by
- Jumping, squatting, stairs, and running
- Rehabilitation focus
- Gradual, supervised strengthening and load management
Why the kneecap area hurts
The patellar tendon runs from the kneecap to the shinbone. It works with the thigh muscles to straighten the knee, so running, jumping, squatting, and climbing stairs can place repeated load through it. Irritation may cause a focused ache or tenderness below the kneecap, with stiffness after rest or soreness following activity.
Pain in the front of the knee can also come from the kneecap joint, a muscle strain, or another knee problem. The exact spot of pain and the movements that reproduce it help a clinician decide whether the tendon is involved.
What may help
Temporarily reduce movements that sharply increase pain, especially repeated jumping or deep knee bending. Keep gentle movement within comfort so the knee does not become stiff. A wrapped cold pack after activity may ease soreness. Once symptoms settle, a physiotherapist can guide progressive exercises that build strength and improve the tendon's ability to handle load.
When to seek medical care
Book an assessment when kneecap-area pain continues, interferes with normal walking, or returns whenever activity increases. Seek urgent help after a sudden injury with a pop, substantial swelling, inability to straighten the knee, or sudden loss of pushing strength. Those findings can point to a more significant tendon or knee injury.
Questions for your doctor
Ask whether the pain pattern fits patellar tendinitis, which exercises are appropriate, and how much activity is reasonable during recovery. Clarify how to progress back to stairs, running, jumping, or sport without a sudden increase in tendon load.
