Does ovarian cysts cause ovulation pain?

An ovarian cyst can stretch the ovary or irritate nearby tissue, causing one-sided pelvic pain. Pain may be felt around the middle of the menstrual cycle, but its timing can vary with the cyst's type and size. Some cysts cause no symptoms at all.

Mid-cycle pain can also happen as part of ovulation without a cyst. Pain from infection, endometriosis, bowel problems, or urinary conditions can feel similar. A clinician uses the timing, location, severity, examination, and sometimes ultrasound to work out the likely explanation.

Arrange a review if pain keeps returning, interferes with daily activity, occurs outside your usual cycle pattern, or comes with bleeding, pressure, or changes in urination or bowel movements. Note the side, timing, severity, and related symptoms before the appointment.

If an ultrasound finds a cyst, ask about its appearance, whether follow-up is needed, and which symptoms should be reported. Treatment depends on the cyst and your symptoms; observation may be appropriate in some situations.

Get urgent medical help for sudden severe pelvic pain, pain with vomiting, fainting, marked weakness, fever, or heavy vaginal bleeding. If pregnancy is possible, urgent assessment is especially important for pelvic pain with bleeding, dizziness, or shoulder-tip pain.
