Does malignant pleural effusion cause shortness of breath?

A pleural effusion is fluid in the space between the lung and chest wall. When the fluid builds up, the lung may have less room to expand during a breath. Breathlessness can be worse when walking, lying flat, or doing ordinary activities, and some people also notice chest pressure or a dry cough. The severity depends on the amount and speed of fluid accumulation, the condition of the lungs, and the underlying illness. Breathlessness has other possible causes, so assessment is needed.

Contact the treating team promptly if breathlessness is new, increasing, or limiting activity. They may examine you and arrange chest imaging to assess the fluid. Treatment can include addressing the underlying cancer and relieving pressure from the fluid, depending on your situation. Sit upright, pace activities, and use prescribed oxygen or medicines exactly as instructed. Do not change cancer treatment without medical advice.

Call emergency services for severe or rapidly worsening breathlessness, blue or grey lips, new confusion, fainting, severe chest pain, or inability to speak comfortably because of breathing difficulty. Sudden breathlessness also needs urgent assessment, especially if it follows a procedure or is accompanied by coughing blood.

Ask what is causing the fluid, whether imaging or drainage is needed, how quickly symptoms should improve after treatment, and which warning signs require emergency help. Discuss sleeping positions, activity limits, and how to use any prescribed equipment.
