How heat cramps happen

Heat cramps are painful, involuntary muscle contractions linked with sweating and physical effort. They may affect the calves, thighs, arms, or abdominal muscles, particularly those working repeatedly. Heat and fluid loss can disturb the conditions muscles need to contract and relax normally. The muscle may become hard or knotted, and soreness can remain after the spasm eases.

A cramp after exercise is not automatically heat-related. Dehydration, overuse, medication effects, and other health problems can produce similar symptoms. The timing, temperature, amount of sweating, and accompanying symptoms help a clinician judge the cause.

What to do during a cramp

Stop exercising and move somewhere cooler. Gently stretch and massage the cramped muscle without forcing it. Sip fluids and rest until the contraction has released and you feel well. Do not return to strenuous activity while cramping, weakness, or nausea continues. If cramps keep recurring, arrange an assessment to review activity, medicines, and other possible contributors.

Warning signs

Seek urgent help if there is confusion, fainting, a seizure, severe weakness, repeated vomiting, or skin that is very hot. Prompt medical advice is also needed when cramps do not improve with cooling and rest, when you cannot keep fluids down, or when urine becomes unusually dark. These signs may point to a more serious heat illness.
