Does heart failure cause shortness of breath?

When the heart cannot circulate blood efficiently, pressure may build in the vessels of the lungs. Fluid can then move into lung tissue, making breathing feel difficult. Breathlessness may first appear during walking, stairs, or other exertion, then occur while resting or lying flat as the condition changes. Waking at night gasping, needing extra pillows, ankle swelling, fatigue, and a persistent cough can accompany it. Lung disease, infection, anaemia, anxiety, and other heart conditions can also cause breathlessness, so the symptom needs clinical context.

Stop and sit upright when breathlessness starts, and use any prescribed plan or treatment exactly as directed. Note what triggered it, how long it lasted, whether lying down worsened it, and whether swelling or weight changed at the same time. Arrange prompt review for new or worsening breathlessness, reduced walking ability, or increasing need to sleep upright. Do not increase or stop heart medicines without advice.

Call emergency services for severe breathlessness, inability to speak comfortably, blue or grey lips, fainting, confusion, chest pain, or sudden deterioration. Sit upright and avoid lying flat while waiting for help. A clinician may check oxygen levels, examine the heart and lungs, and use an electrocardiogram, imaging, or blood tests to identify fluid build-up and other causes.

Ask whether your breathlessness reflects fluid congestion, a rhythm change, lung disease, infection, anaemia, or another problem. Ask how to monitor symptoms at home, what activity is appropriate, and which changes require same-day or emergency assessment.
