Does genitourinary syndrome of menopause cause bleeding after sex?

Lower oestrogen after menopause can affect the vulva, vagina, bladder, and urethra. Vaginal tissue may become drier, thinner, less flexible, and more easily irritated. Friction during sex can then cause tiny surface breaks and spotting. Burning, soreness, dryness, urinary urgency, or pain during sex may occur alongside the bleeding.

Bleeding after sex also has other possible causes, including infection, cervical changes, polyps, and less common but important conditions. A symptom pattern cannot establish the cause, particularly after menopause, so an examination is worthwhile even when dryness seems likely.

Arrange a GP or gynaecology appointment for bleeding after sex, especially if it has happened more than once or you have gone through menopause. A clinician may examine the vulva, vagina, and cervix and discuss swabs or screening according to your symptoms and history.

Until reviewed, use a water-based lubricant during sex and allow more time for arousal. Vaginal moisturisers may help ongoing dryness. A clinician can discuss local oestrogen or other treatments when appropriate; do not insert products that cause stinging or use unprescribed hormones.

Seek urgent medical help for heavy bleeding, dizziness, fainting, severe pelvic pain, fever, or feeling weak. Contact a clinician promptly for bleeding that recurs, occurs without sex, or is accompanied by unusual discharge, a pelvic mass, or unexplained weight change.
