Does exercise therapy treat spondylolysis?

Short answer
Exercise therapy may help symptoms and function in spondylolysis, but the plan must respect healing and avoid repeated spinal stress.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Early focus
- Reduce painful loading and protect movement quality
- Later focus
- Progress trunk, hip, and activity-specific strength
Understanding the condition
Spondylolysis is a stress injury or defect in the pars region of a vertebra, commonly in the lower back. Exercise is not a way to force the defect to close. During an irritable phase, a clinician may reduce painful extension, twisting, and high-impact loading before introducing controlled strengthening. Later work can include deep trunk muscles, hips, posture, and sport-specific movement. The right pace depends on your examination, age, activity, symptoms, and any associated vertebral movement.
Building back activity
Have a physiotherapist or doctor identify the movements that reproduce your pain and set clear stages for return to work or sport. Practise only the prescribed exercises, keep the spine in the recommended position, and stop if pain becomes sharp, spreads, or lingers increasingly after a session. Avoid testing recovery with sudden heavy lifting or repeated back arching. More detail is available in [Exercise Therapy](/treatments/physiotherapy-and-rehabilitation/exercise-therapy/).
Warning signs
Seek urgent care for new bladder or bowel changes, numbness around the groin, or marked or progressive weakness in a leg. Contact your clinician promptly if pain follows a major injury, becomes severe at rest, or continues to worsen despite activity modification. Leg pain, tingling, or weakness can indicate nerve involvement and deserves review.
Useful questions
Ask whether your symptoms suggest an active stress injury, which activities should be paused, and what signs show that loading can increase. Ask when follow-up assessment or imaging is needed before returning to sport.

Questions people ask
Can I exercise with spondylolysis?
Often, carefully selected low-load movement is possible, but painful extension and impact may need temporary limits set by a clinician.
Does exercise heal the pars defect?
Exercise supports controlled recovery and function; it does not guarantee that a bony defect will close.
