Does endometriosis cause painful periods?

Endometriosis involves tissue similar to the womb lining growing outside the womb. During a period, nearby inflammation can irritate pelvic tissues and cause cramping or deeper pain. Pain may begin before bleeding, continue through the period, or spread to the back or legs.

Endometriosis-related pain may occur alongside heavy bleeding, pain during sex, pain with bowel movements, urinary discomfort, or difficulty becoming pregnant. Those features can help a clinician decide what to investigate, but no single symptom confirms the condition.

Keep a cycle diary showing when pain begins, how long it lasts, how it affects sleep or work, and whether medicines help. Share any heavy bleeding, bowel or bladder symptoms, pain outside periods, or fertility concerns during an appointment.

A clinician may discuss an examination, imaging, pain relief, hormonal options, or referral. The right choice depends on symptoms, examination findings, and whether you are trying to become pregnant.

Arrange an appointment if period pain is worsening, limits normal activities, starts after years of manageable periods, or does not improve with usual measures. Seek urgent care for severe sudden pain, fainting, fever, vomiting, or very heavy bleeding with weakness.
