Why the calf may hurt

The lower leg contains groups of muscles, nerves, and blood vessels enclosed by firm tissue. In compartment syndrome, pressure within one of these spaces can rise and restrict normal circulation and nerve function. Pain may feel deep, tight, or out of proportion to the activity or injury. Acute symptoms can follow a fracture, crush injury, bleeding, or very tight bandaging. Chronic exertional symptoms may build during exercise and settle with rest. Calf pain has other possible causes, so its presence alone does not confirm compartment syndrome.

What to do now

Stop the activity that brings on the pain and do not try to train through increasing tightness, swelling, weakness, or altered sensation. If symptoms began after an injury, keep the leg in a comfortable position and arrange prompt medical assessment. Do not loosen or remove a medical dressing without advice, but tell the treating team if it feels unusually tight. A clinician may examine muscle firmness, movement, sensation, and circulation before deciding what care is needed.

When urgent care is needed

Seek emergency care immediately for severe or rapidly increasing calf pain, a tense or swollen lower leg, pain when the foot or toes are gently moved, new numbness or tingling, weakness, pale or cool skin, or difficulty moving the foot. After trauma or surgery, these changes should not wait for a routine appointment. Acute compartment syndrome can worsen quickly and requires urgent assessment.
