Does chronic disease review treat chronic kidney disease?

Chronic kidney disease often needs continuing observation because kidney function, blood pressure and related health problems can change over time. A review may cover recent blood and urine results, blood pressure, swelling, diabetes if relevant, diet, smoking and medicines. Some medicines need checking when kidney function changes, and certain pain medicines may be unsuitable for some people. The appointment is a monitoring and care-planning step, not a way to reverse established kidney damage in one visit.

Bring your medicine list, home blood-pressure readings if you take them, and questions about fluids, salt, pain relief or supplements. Tell the clinician about reduced urine, ankle swelling, nausea, itching, fatigue or changes in appetite. Do not alter prescribed medicines or fluid intake on your own; the right advice depends on kidney function and other conditions. Keep scheduled blood and urine checks so changes can be compared over time.

Request medical advice if swelling, breathlessness, urine changes or blood pressure problems are new or worsening. Seek urgent help for severe breathlessness, chest pain, confusion, fainting, or very little urine with feeling acutely unwell. A sudden deterioration should not wait for a routine review.
