Does adenomyosis cause painful periods?

In adenomyosis, tissue similar to the womb lining is found within the uterine muscle. Hormonal changes during the cycle can make these areas swell and trigger contractions in the uterus. Pain may begin before bleeding, continue through the period, or feel as deep aching, cramping, or pressure in the pelvis. It may occur with heavy or prolonged bleeding, though symptoms differ between people.

Painful periods are not specific to adenomyosis. Endometriosis, fibroids, pelvic infection, and other conditions can cause similar symptoms. The timing, severity, examination, and any changes over time help guide assessment.

Record when pain starts, where it is felt, how long it lasts, and whether it affects work, sleep, bowel movements, sex, or urination. Note bleeding changes and medicines you have tried. A clinician may discuss an examination and pelvic imaging. Treatment is chosen around your symptoms, health history, and pregnancy plans, rather than the pain description alone.

Arrange medical review when period pain is worsening, stops you doing normal activities, begins after years of manageable periods, or does not improve with your usual measures. Seek urgent help for sudden severe pelvic pain, fainting, fever, vomiting, or pain with possible pregnancy. New pain with unusual discharge or bleeding also needs prompt assessment.
