Does an ACL tear cause knee swelling?

Short answer
Knee Swelling is a common presentation of ACL Tear and may develop soon after the injury.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Why it happens
- Irritation or bleeding inside the knee joint
- What to monitor
- Rapid swelling, locking, instability, or difficulty bearing weight
What the swelling can mean
The ACL sits inside the knee joint. When it is injured, bleeding or irritation inside the joint can make the knee look puffy and feel tight. You may notice difficulty bending the knee, stiffness after sitting, pain when putting weight through the leg, or a sense of looseness. Swelling can also occur with a meniscus injury, a kneecap problem, or other sprain, so its presence does not identify one cause by itself. The timing, injury mechanism, examination findings, and sometimes imaging help a clinician work out what has happened.
How to manage it initially
Pause sport and activities that involve turning or jumping. Elevate the leg when resting, and place a cold pack wrapped in a towel over the knee for short intervals. Keep movements gentle and within comfort; do not massage a freshly swollen knee or repeatedly bend it to check the range. If walking is painful or the knee feels unstable, arrange a medical assessment and ask whether temporary support is suitable. Use medicines only as directed by a qualified professional, especially if you have other health conditions.
When to seek care
Get urgent help if swelling follows a major injury and you cannot stand, the knee is visibly misshapen, or the foot is numb, pale, or unusually cold. Prompt review is needed for fast swelling, severe pain, fever, marked redness, or a knee that locks and will not straighten. A clinician can check circulation, movement, stability, and possible associated injuries.
Questions for your doctor
Useful questions include: Does the swelling pattern suggest an internal knee injury? Do I need imaging? How much weight can I place on the leg? Should I use a brace, and when can I begin guided rehabilitation?

Questions people ask
Can an ACL tear cause swelling without much pain?
It can, although symptoms vary. New swelling after an injury still deserves assessment, particularly if movement or walking is limited.
Is every swollen knee an ACL tear?
No. Several injuries and inflammatory conditions can swell the knee. A clinical assessment is needed to identify the cause.
