Diarrhea, vomiting, chills and body aches: what to know

Short answer
Diarrhea, vomiting, chills and body aches commonly occur when an infection irritates the digestive system or activates the body's wider immune response.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Main concern
- Fluid and salt loss from vomiting and diarrhea
- Helpful detail
- Note fever, urine output, travel and recent food or water exposure
- Immediate priority
- Take frequent small sips of water or oral rehydration solution
What should you know about these symptoms?
A stomach infection or foodborne illness can cause loose stools and vomiting, while fever-related chills and muscle aches reflect the body's response. Respiratory infections such as influenza or COVID-19 can also include digestive symptoms, particularly alongside fever, fatigue, cough or a sore throat.
What the combination may mean
The pattern does not identify one illness by itself. The timing of symptoms, recent food or water exposure, travel, contact with someone unwell, fever, abdominal pain and urine output help a clinician narrow the possibilities.
Travel or mosquito exposure can make illnesses such as malaria or dengue fever relevant. Contact with livestock or unpasteurised dairy products may prompt consideration of brucellosis. Persistent body aches without an acute stomach illness may need a different assessment, including conditions such as fibromyalgia, hypothyroidism or Long COVID.
Vomiting and diarrhea can remove water and salts faster than you replace them. Dehydration may cause thirst, a dry mouth, dizziness, weakness, dark urine or passing urine less often.
What to do at home
Take frequent small sips of water or an oral rehydration solution. If vomiting makes drinking difficult, pause briefly and restart with small amounts. Once fluids stay down, choose simple foods such as rice, toast, bananas, soup or yogurt if tolerated; avoid alcohol and foods that clearly worsen symptoms.
Rest, wash your hands carefully and avoid preparing food for other people while you are unwell. Do not take antibiotics left over from another illness. A pharmacist or clinician can advise whether an anti-diarrheal or fever medicine is suitable, especially for children, pregnancy, older adults or people with chronic conditions.
When to seek medical care
Seek urgent care for severe or worsening abdominal pain, confusion, fainting, trouble breathing, a stiff neck, a seizure, blood or black material in vomit or stool, or inability to keep fluids down. Get prompt advice if there are clear signs of dehydration, a very high fever, or symptoms after recent travel to a malaria-risk area.
Arrange medical assessment when symptoms are not improving, keep returning, or affect an infant, an older adult, someone pregnant, or a person with a weakened immune system. A clinician may examine hydration, abdomen and vital signs and decide whether blood, stool or other tests are needed.
Questions for your doctor
Ask: What is the most likely source of these symptoms? How can I replace fluids and salts safely? Do I need testing for an infection? Which medicines should I avoid? What changes should prompt urgent care? Mention the symptom start time, number of vomiting or diarrhea episodes, measured temperature, urine changes, travel, food exposures, medicines and relevant medical conditions.

Why do body aches happen with vomiting and diarrhea?
Body aches can accompany the immune response to an infection, while fluid and salt loss can add weakness and muscle discomfort.
When should I worry about dehydration?
Warning signs include marked thirst, a dry mouth, dizziness, weakness, dark urine or passing urine less often, especially when fluids will not stay down.
