Diarrhea causing nappy rash: care and warning signs

Liquid stool spreads across a larger area and may remain against the skin between changes. Repeated wiping adds friction, while digestive enzymes and moisture weaken the protective outer layer. The result can be red, sore, shiny, or raw skin on the buttocks and around the anus. If the rash reaches the folds or develops small spots around its edge, yeast may be contributing. Diarrhoea can also lead to dehydration, which is a separate concern from the rash.

Change the nappy as soon as it is wet or soiled. Rinse with lukewarm water or use gentle fragrance-free wipes, then pat dry carefully. Apply a thick protective barrier recommended for babies after each change, and allow brief supervised air exposure when possible. Avoid talc, scented products, vigorous rubbing, and tight nappies. Continue appropriate fluids and follow advice for managing the diarrhoea. If the skin is not improving or the folds are involved, a clinician may decide whether a topical antifungal treatment is suitable.

Contact a healthcare professional if the rash is severe, blistered, bleeding, infected-looking, or not settling with careful care. Seek prompt help if the child has fever, repeated vomiting, blood in the stool, severe pain, unusual sleepiness, poor drinking, a dry mouth, or fewer wet nappies. Babies who seem unwell or children with ongoing diarrhoea should be assessed without waiting for the skin to worsen.
