Diarrhea and miscarriage: what to know

Loose stools during pregnancy can follow a stomach infection, food intolerance, dietary change or a medicine. Diarrhea is not a defining sign of miscarriage. Miscarriage more often involves vaginal bleeding, cramping or passing tissue, although symptoms vary and some losses are found during an ultrasound. Repeated watery stools may reduce fluid and salts, which can leave you dizzy, weak or passing less urine. A clinician can assess your symptoms, pregnancy history and hydration. A transvaginal ultrasound may help check the pregnancy and uterus when miscarriage is suspected.

Take frequent small sips of water or an oral rehydration drink. Choose simple foods when you feel able, and avoid foods that clearly worsen the diarrhea. Rest, wash your hands carefully and do not start an anti-diarrhea medicine without checking that it is suitable in pregnancy. Note when the diarrhea began, how often it occurs, whether you have fever or vomiting, and any bleeding or pain. If a miscarriage is diagnosed, your clinician can explain options such as medical management or surgical management, based on your symptoms and examination.

Contact urgent maternity or emergency care for heavy vaginal bleeding, severe or one-sided abdominal pain, fainting, confusion, fever, blood in the stool, repeated vomiting, or inability to keep fluids down. Seek advice promptly if you are passing very little urine, feel markedly dizzy, or have diarrhea that is not settling. If you are pregnant and worried about bleeding or pain, do not wait for diarrhea to explain it; describe the pregnancy symptoms directly. Emergency assessment may include examination, blood tests, fluid treatment and ultrasound.

Ask what is most likely causing the diarrhea, whether you need hydration support, which medicines are safe, and whether bleeding or cramps require an ultrasound. If miscarriage is confirmed, ask how each management option affects follow-up and what warning signs should bring you back.
