Diagnosing overactive bladder: what to know

How the structures involved in Overactive Bladder differ from normal

Short answer

Overactive bladder is diagnosed from urinary urgency and frequency after a clinician reviews symptoms and excludes infection or other causes.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Core symptom
A sudden, difficult-to-defer urge to urinate
Useful record
A bladder diary

How the assessment works

Tell the clinician how often you pass urine, whether you wake at night, whether urgency causes leakage, and whether there is burning or pelvic pain. A bladder diary can show fluid intake, voiding times, urgency and leakage patterns. Urine testing may check for infection or blood, while an examination can assess pelvic organs, nerves and pelvic-floor function. Symptoms may overlap with infection, constipation, medicines, diabetes or bladder-emptying problems, so the diagnosis is not based on urgency alone.

Preparing for care

Keep a diary for a few days if your clinician suggests it, and bring a list of medicines, supplements and drinks. Do not drastically restrict fluids without advice. If the initial assessment does not explain your symptoms, urodynamic testing may measure bladder pressure, flow and storage during filling and emptying. Treatment planning may include bladder training and pelvic-floor rehabilitation after the cause has been clarified.

When to seek prompt help

Seek prompt medical care for blood in the urine, fever with urinary symptoms, pain in the side or back, inability to pass urine, new weakness or numbness, or sudden loss of bladder control. Urgency with severe pain or vomiting also needs assessment. These features may point to a problem requiring a different response from routine overactive-bladder care.

Questions for your doctor

Ask which causes should be excluded, whether you need urine testing or a bladder diary, and what would make urodynamic testing useful. Discuss caffeine, constipation, medicines, pelvic-floor symptoms and whether bladder training is suitable for your pattern.

Questions people ask

No. Urgency and frequent urination can occur with or without urgency-related leakage.

Why is a urine test sometimes needed?

It helps check for infection, blood and other findings that can mimic or accompany bladder symptoms.
