What should you know about diabetes and gout?

Short answer
Diabetes and gout can occur together, so joint symptoms should be assessed while blood sugar, kidney health, medicines and uric acid are reviewed.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Gout
- Urate crystals cause inflammation in a joint.
- Diabetes
- Illness and medicines can affect glucose management.
- Red flag
- A hot swollen joint with fever needs same-day assessment.
How the conditions relate
Gout happens when urate crystals trigger inflammation in a joint. It often causes sudden pain, warmth, redness and swelling, commonly in a foot, ankle or knee, although other joints can be involved. Diabetes does not prove that a painful joint is gout; infection, injury and other inflammatory conditions can look similar. Diabetes and gout may share contributors such as reduced kidney function, medication effects and metabolic health, so the full history matters.
A clinician may examine the joint, review medicines and kidney function, and order blood tests or joint-fluid testing when the diagnosis is uncertain. Do not change diabetes or gout medicines without advice.
Managing the bigger picture
During a flare, rest the affected joint, elevate it when comfortable and use only pain or anti-inflammatory medicines that are safe for your diabetes, kidneys, stomach and other prescriptions. After the flare, discuss a long-term plan for gout, glucose management, hydration, weight goals and regular monitoring. Chronic Kidney Disease and Obesity may be relevant parts of that conversation. When to seek care promptly
Seek same-day medical care for a hot, red, very swollen joint, especially with fever, chills, feeling unwell, a wound, or poorly controlled diabetes, because joint infection needs rapid assessment. Urgent help is also needed if pain follows an injury, you cannot bear weight, or the foot becomes pale, cold, numb or weak. Severe vomiting or dehydration can make diabetes harder to manage.
Questions for your doctor
How will we confirm gout rather than infection or injury? Could any of my medicines affect urate or glucose? How should I manage medicines during a flare? Should kidney function and urate be monitored? What symptoms mean I should attend urgent care?

Questions people ask
Does diabetes cause every episode of joint swelling to be gout?
No. Infection, injury and other inflammatory conditions can cause similar symptoms and need different treatment.
Can I take my usual pain medicine during a gout flare?
Check first, because kidney function, stomach health, blood pressure, glucose control and other medicines affect which options are safe.
