Dermoid cyst scalp treatment: what to know

A pilar cyst is a sac beneath the scalp skin, often arising from a hair follicle. It may feel smooth, firm, and rounded, and the scalp may have more than one. A clinician checks the lump directly because an inflamed cyst can become tender, swollen, or warm, while an uncomplicated cyst may cause little discomfort. The term dermoid cyst is sometimes used broadly for a cyst present from development; the exact diagnosis matters when choosing treatment.

Examination usually considers whether the lump is mobile, attached to deeper tissue, irritated, or infected. A sudden change does not establish a diagnosis by itself, so avoid squeezing or puncturing it at home.

If the cyst is small and quiet, a clinician may discuss observation and simple scalp care. Persistent irritation, repeated inflammation, rapid enlargement, or concern about the diagnosis may lead to a procedure. Removal aims to take out the cyst wall as well as its contents, because leaving the wall behind can allow the lump to return. The method depends on the cyst and the surrounding skin; local anaesthetic may be used, followed by wound care and instructions about washing the scalp.

Do not try to drain it yourself. Tell the clinician about blood-thinning medicines, allergies, diabetes, or previous problems with wound healing before a procedure.

Arrange a clinical assessment for a new scalp lump, especially if it is growing, painful, repeatedly irritated, or affecting brushing, sleeping, or headwear. Seek prompt medical care if redness is spreading, the area feels hot, pus is present, fever develops, or pain escalates quickly. A lump that bleeds, ulcerates, becomes fixed, or changes in a way you cannot explain also needs review.

Ask whether the lump appears to be a pilar cyst or another type of growth, whether observation is reasonable, and what complete removal would involve. Clarify expected wound care, activity restrictions, signs of infection, and whether the removed tissue will be examined.
