What rhabdo means

Muscle cells can be damaged by unusually hard or unfamiliar exercise, a crush injury, prolonged pressure, heat illness, seizures, infection or some medicines and substances. As muscle breaks down, its contents enter the bloodstream. The pattern may include severe muscle pain, marked weakness, swelling and urine that looks dark or tea-coloured, although symptoms vary and the urine can appear normal. Ordinary muscle soreness after exercise tends to follow a familiar effort and improve with rest; rhabdomyolysis is more concerning when pain, weakness or swelling feels extreme or does not fit the activity.

What to do if you suspect it

Stop strenuous exercise and move to a cooler place if heat may be involved. Do not try to train through severe soreness, weakness or swelling. Seek urgent medical assessment, especially when urine becomes dark, urination decreases, or symptoms follow a major injury. Clinicians may examine you and check blood and urine to assess muscle breakdown and kidney function. Treatment can include monitored fluids and management of the trigger; the plan depends on your symptoms and test results. Avoid restarting intense activity until a clinician gives you a gradual return plan.

When to seek care

Seek urgent care now for severe muscle pain with weakness, rapidly increasing swelling, dark urine, very little urine, confusion, fainting or trouble breathing. After heat exposure, collapse, inability to keep fluids down or a reduced level of alertness is an emergency. Do not wait for all signs to appear: rhabdomyolysis can affect the kidneys before you feel dramatically ill. Mild soreness alone after exercise is not proof of rhabdo, but symptoms that are unusually intense, persistent or accompanied by weakness deserve medical advice.
