Dead bone in osteomyelitis: what it means and what to do

Short answer
Dead bone in osteomyelitis is bone tissue damaged by infection and reduced blood flow, requiring medical assessment and treatment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Key issue
- Infection can damage bone and its blood supply.
- Assessment
- Examination, imaging, and sometimes tissue sampling are used.
What dead bone means
A bone infection can make nearby tissue inflamed and impair its blood supply. The affected fragment may separate from healthier bone and cannot respond to treatment in the same way as well-perfused tissue. This does not describe ordinary dry skin or a simple bruise. It may be associated with persistent pain, swelling, drainage, warmth, reduced movement, or a wound that does not close.
Clinicians use an examination and imaging to judge the extent of infection. Blood tests or a sample from the affected area may help identify the organism and guide antibiotics. The appearance of a wound alone cannot establish whether bone is dead.
What to do next
Arrange prompt review with an orthopaedic or infectious-disease clinician, especially if you have a wound near bone, recent surgery, a fracture, or an implanted device. Treatment may involve targeted antibiotics, wound care, removal of infected or non-viable tissue, and attention to circulation and nutrition. Do not stop prescribed antibiotics early or probe the wound yourself.
If infection has left a gap or weakened an area after damaged tissue is removed, reconstruction may be discussed. Read about [Bone Grafting](/treatments/trauma-and-fracture-care/bone-grafting/) as one possible rebuilding approach.
When to seek urgent care
Seek urgent medical help for rapidly increasing pain or swelling, spreading redness, foul drainage, fever with chills, confusion, marked weakness, or a limb that becomes pale, cold, numb, or difficult to move. A worsening wound or new drainage over metalwork also needs prompt assessment. Do not wait for symptoms to become severe if you have diabetes, poor circulation, or immune suppression.
Questions for your doctor
Ask which imaging shows the affected bone, whether a culture is needed, how long treatment may take, and whether surgery is being considered. Ask how to protect the limb, how the wound should be dressed, and which changes should trigger same-day contact.

Questions people ask
Can dead bone heal with antibiotics alone?
Antibiotics may control infection, but poorly supplied or separated bone may need surgical assessment and removal.
Is drainage from a bone infection urgent?
New or worsening drainage should be assessed promptly, particularly with fever, increasing pain, or redness.
