Dark underarms and PCOS treatment: what to know

Some people with PCOS develop darker, thicker, velvety skin in folds such as the underarms. This pattern is known as acanthosis nigricans and can be associated with insulin resistance, but darkening can also follow shaving, friction, deodorant irritation, or a rash. A dark underarm is not by itself a diagnosis of PCOS; the wider pattern and medical history matter.

Book a medical review if the change is new, spreading, or velvety. Your clinician may consider metabolic assessment and PCOS care alongside a skin examination. Use gentle cleansing, avoid scrubbing, and reduce friction while the cause is clarified. Dermatology treatments such as prescription pigment therapy, a chemical peel, or laser toning may be discussed for selected cases, but irritated or thickened skin needs appropriate assessment first.

Arrange care for fast change, widespread fold involvement, marked thickening, itch, pain, cracking, or a persistent rash. Seek medical advice if darkening occurs with irregular periods, increased facial hair, acne, unexplained weight change, unusual thirst, or frequent urination. These features help a clinician decide whether endocrine, metabolic, inflammatory, or cosmetic factors are involved.

Ask whether the texture suggests acanthosis nigricans, whether PCOS or glucose-related checks are appropriate, and which skin treatment can be used alongside your current medicines.
