What a cystocele can feel like

The bladder sits behind the front wall of the vagina. When the supporting tissues stretch or weaken, that wall can move downward and create a sensation of heaviness, pressure, or a vaginal bulge. You may notice symptoms after standing, lifting, coughing, or late in the day, and they may ease when lying down. Bladder symptoms can include leakage with exertion, needing to urinate often, a slow stream, or a feeling that the bladder has not emptied. Constipation and changes after pregnancy, childbirth, menopause, or pelvic surgery can contribute to pelvic support problems, but a clinician should assess the individual pattern.

How it is managed

A clinician usually reviews your symptoms and performs a pelvic examination, sometimes asking you to bear down. Keep a record of pressure, leakage, emptying difficulty, and activities that trigger symptoms. Pelvic-floor physiotherapy and avoiding straining may improve support and control. A vaginal pessary can provide removable support and may be fitted to your anatomy. When symptoms substantially affect daily life or conservative care is not enough, pelvic floor repair surgery may be an option to discuss. The right plan depends on your examination, health, goals, and future pregnancy plans.

When to seek care

Book a clinical review for a new vaginal bulge, urinary leakage, recurrent bladder infections, or pressure that interferes with work, exercise, sleep, or sex. Seek urgent care if you cannot urinate, develop severe pelvic pain, have fever with urinary symptoms, or notice heavy bleeding. Do not try to push painful tissue back repeatedly; irritated or injured tissue needs examination.
