Croup virus treatment: helping a child breathe comfortably

Croup is usually caused by a virus, so antibiotics do not treat the cause. Symptoms come from swelling around the larynx and nearby upper airway. A clinician will consider the child’s breathing at rest, ability to drink, alertness, and the sound of the breathing. Mild symptoms may settle with calm observation and fluids. More pronounced airway swelling may need a steroid medicine prescribed by a clinician. A child with significant breathing difficulty may need an urgent assessment and treatment in a medical setting.

Hold or sit the child upright, use a quiet voice, and reduce crying because distress can increase the work of breathing. Offer cool fluids in small amounts when the child can swallow safely. Keep the room smoke-free. Do not use steam, force food or drinks, or give leftover prescription medicine. If a clinician has provided a croup plan, follow its instructions exactly. Nebuliser treatment may be considered for severe symptoms, but it should be given under medical guidance rather than started at home without advice.

Seek emergency care if the child has noisy breathing while resting, visible pulling in at the ribs or neck, pauses in breathing, blue or grey lips, drooling with trouble swallowing, or marked drowsiness. Contact a clinician promptly when symptoms are getting worse, the child cannot drink, or the cough and breathing noise return after treatment. Ask which symptoms should improve first, how to give any prescribed medicine, whether a follow-up is needed, and exactly where to seek help if breathing worsens. Tell the clinician about allergies, previous airway episodes, current medicines, and how much the child has been drinking.
