Paediatric croup treatment: what to expect

Croup causes swelling near the voice box, so treatment is guided by the child's breathing when settled, voice, cough, hydration, and alertness. A mild barking cough may need comfort measures and observation, while stridor or increased effort calls for medical assessment.

A clinician may prescribe medicine to reduce airway swelling. Nebuliser treatment may be considered when rapid relief is needed for significant breathing symptoms. Antibiotics do not treat the viral causes that commonly produce croup unless another infection is identified.

Hold or sit the child upright, speak quietly, and avoid upsetting examinations when possible. Offer small, frequent drinks. Keep smoke, aerosols, and strong fragrances away from the child. Give prescribed medicine exactly as directed and do not use adult medicines or steam inhalation.

Arrange a child health consultation for a new episode you cannot assess, symptoms that return, or questions about dosing. Note whether noisy breathing occurs during calm rest, activity, or crying, since that detail helps guide treatment.

Seek emergency help for severe breathing effort, persistent stridor at rest, chest or neck pulling in, blue or grey lips, drooling with difficulty swallowing, pauses in breathing, or a child who is unusually drowsy or hard to wake.

Get prompt medical advice if drinking drops, urine becomes much less frequent, the cough is worsening, or the child needs repeated reassurance to breathe comfortably. Keep the child upright while arranging care and take prescribed medicines with you.

Ask which symptoms make medicine necessary, how quickly it should help, what to do if the dose is vomited, and whether a nebuliser treatment is appropriate. Ask for a clear overnight plan and the exact signs that mean emergency care.
