Croup disease treatment: what parents should know

Croup is usually a viral inflammation around the voice box and upper windpipe. Swelling there creates a hoarse voice, a barking cough, and sometimes a harsh sound when breathing in. Crying can make the noise and effort worse, so reassurance and an upright position are useful while arranging advice. A clinician may listen to breathing, assess the child’s alertness and hydration, and decide whether medicine to reduce swelling is needed. Antibiotics do not treat the usual viral cause.

Hold the child upright and speak quietly. Offer small, frequent drinks if swallowing is comfortable. Keep the air around the child free from smoke and avoid forcing food or fluids during a coughing spell. Do not use steam or give cough medicines unless a clinician has specifically advised them. A child-health assessment can help when the cough is new, symptoms are recurring, or you are unsure whether the breathing noise is stridor. In a medical setting, a nebuliser treatment may be considered for significant airway narrowing.

Seek emergency help if the child struggles to breathe, has persistent noisy breathing while resting, pulls in at the ribs or neck, cannot speak or cry normally, becomes unusually sleepy, drools because swallowing is difficult, or develops blue or grey lips. Keep the child calm during transfer. A mild barking cough without breathing difficulty can often be discussed with a clinician, but worsening symptoms at night deserve prompt advice.

Ask what signs show that airway swelling is worsening, whether prescribed medicine should be repeated, how to monitor breathing during sleep, and when the child may return to nursery or school. Mention fever, reduced drinking, previous breathing episodes, allergies, and any medicine already given.
