Croup cough without fever: what to know

Croup affects the area around the voice box and may cause a barking cough, hoarseness, or a high-pitched sound when breathing in. Fever may be absent or mild. Other causes of cough can sound similar, including irritation, wheeze, or infection lower in the airways. No fever does not prove that the illness is minor, and the child's breathing when settled gives more useful information.

Hold the child upright and speak quietly to reduce distress. Offer sips of fluid when they can swallow comfortably. Keep the air comfortable and avoid smoke, strong scents, and hot steam. Do not give someone else's inhaler or leftover prescription medicine. Check the child periodically, including while resting, for changes in breathing, colour, hydration, and responsiveness.

Get urgent help for noisy breathing while the child is calm, skin pulling in around the ribs, pauses in breathing, trouble swallowing, drooling, blue or grey lips, marked drowsiness, or inability to cry or speak normally. Seek same-day advice when symptoms are persistent, worsening, or affecting drinking, even without fever. A clinician can distinguish upper-airway croup from other causes and prescribe treatment when appropriate.

Ask what breathing sounds should prompt immediate help, how to position and monitor the child overnight, and whether an examination is needed despite the lack of fever. Share when the cough began and whether it changes with rest or activity.
