What should you know about croup cough in infants?

Croup involves swelling near the voice box and upper airway. An infant may have a bark-like cough, hoarseness, a runny nose, or a harsh sound when breathing in. Symptoms can become more noticeable when the child is upset. Bronchiolitis, a common cold, asthma-like wheeze, reflux, or another infection can sound different but overlap in young children, so the sound should be considered alongside breathing effort and behaviour.

Watch the chest and neck rather than judging the cough alone. Pulling in between the ribs or below the neck, pauses in breathing, unusual sleepiness, poor feeding, or fewer wet nappies can signal that the infant is struggling.

Keep the infant calm and hold them upright if that is comfortable. Offer usual breastfeeds or formula in small, frequent amounts if swallowing is easy. Avoid steam, smoke, and non-prescribed cough medicines. Stay nearby and observe the breathing pattern, colour, feeding, and responsiveness. Contact a clinician for advice even when symptoms seem mild if the infant is very young or the cough is new and harsh.

Call emergency services for struggling or paused breathing, blue or grey colour, marked chest indrawing, limpness, inability to feed, drooling with difficulty swallowing, or a harsh breathing noise that continues while the infant is calm. Seek urgent medical advice for worsening cough, fever with reduced responsiveness, repeated vomiting, or much less urine. Do not wait for the cough to settle if breathing looks laboured.

Ask how to recognise noisy breathing, chest indrawing, and dehydration in your infant. Ask whether an examination is needed today, what feeding pattern to monitor, and which symptoms require emergency help. Mention birth history, recent illness, known allergies, and any medicine already given.
