Croup cough but no other symptoms

Croup is an upper-airway inflammation that often produces a barking or seal-like cough. A child may appear otherwise well, especially early in an illness. The cough can sound worse at night or after crying because the upper airway becomes narrower when the child is upset. A rough voice or noisy breathing while breathing in can occur, but their absence does not confirm or exclude croup.

An isolated cough can also come from another airway irritation, such as a common cold, allergy, reflux or asthma. The sound alone cannot identify the cause. Watch the child’s breathing when calm, their ability to drink, and whether they are alert and interacting normally.

Keep the child calm and upright, offer small amounts of fluid, and avoid smoke, strong scents and unnecessary exertion. Check them regularly, including during sleep, for changes in breathing or behaviour. Do not use steam or give cough medicines unless a clinician has specifically advised them; steam can cause burns and many cough medicines are unsuitable for young children.

Arrange clinical advice if the barking cough persists, returns repeatedly, or you are unsure whether the breathing is normal. A clinician may decide whether treatment is needed after hearing the cough and assessing the child.

Seek emergency help if the child struggles to breathe, has pulling in around the ribs or neck, develops blue or grey lips, cannot swallow saliva, becomes unusually sleepy, or is difficult to wake. Noisy breathing while resting is also an urgent warning sign. Keep the child upright and calm while help is arranged.

Ask: Does this sound like croup or another cause of cough? What breathing changes should make us seek urgent help? Should we check the child during the night, and is any prescribed medicine appropriate? Mention when the cough began, whether it is worse at night, and any exposure to illness, smoke or allergens.
