Croup cough and fever: what to watch for

Croup causes swelling in the upper airway and may follow a viral infection. Fever can be part of that infection, while the barking cough and hoarse voice come from the swollen voice-box area. A fever reading does not show how open the airway is, so watch breathing separately. Cough, fever, congestion, and tiredness can also occur with other respiratory illnesses.

A child who is responsive and drinking may need monitoring and advice, whereas a child who is hard to wake, breathing with effort, or unable to drink needs urgent assessment regardless of the temperature.

Keep the child comfortable, lightly dressed, and offered frequent fluids. Use fever medicine only as directed for the child’s age and weight by a healthcare professional or the product instructions; never combine products without checking their ingredients. Keep the child upright and calm during coughing episodes, avoid smoke, and record the temperature, fluid intake, wet nappies or urination, and breathing changes for the clinician.

Seek emergency care for stridor at rest, severe chest indrawing, blue or grey lips, pauses in breathing, drooling with inability to swallow, confusion, or marked sleepiness. Prompt medical advice is needed when fluids are poorly tolerated, urine is much less frequent, the fever is accompanied by a rapidly worsening cough, or the child is not responding as usual. Breathing signs take priority over the fever number.
