Understanding CRMO

CRMO stands for chronic recurrent multifocal osteomyelitis. It is an autoinflammatory condition: the immune system drives inflammation in bone, and symptoms may appear in different places or return after quieter periods. Children and adolescents are often affected, although inflammatory bone disease can be considered at other ages.

Commonly affected areas include the collarbone, spine, pelvis, or long bones. Pain, tenderness, warmth, swelling, and reduced movement may occur. Skin or bowel inflammatory conditions can coexist. Because infection, injury, tumour, and other inflammatory disorders can look similar, diagnosis usually combines examination, imaging, blood tests, and sometimes a biopsy.

Living with and managing it

Keep a symptom record showing pain location, swelling, stiffness, fever, skin changes, and activity limits. Attend follow-up visits so bone inflammation and growth or joint function can be monitored. Treatment is chosen by the specialist and may include anti-inflammatory or other immune-directed medicine, along with activity adjustment and physiotherapy for strength and mobility.

Protect painful areas from impact while maintaining comfortable movement. Tell the team about dental work, new medicines, infections, and changes in walking or posture. Bone grafting is a separate surgical treatment used for selected bone problems, not a routine answer for every CRMO flare.

When to seek urgent care

Contact the treating team promptly for rapidly increasing bone swelling, new inability to use a limb, severe spinal pain, new weakness or numbness, or persistent fever. Emergency care is needed for confusion, collapse, severe breathing difficulty, or sudden loss of bladder or bowel control. A new hot, red, very painful area with fever needs assessment because infection still requires consideration.
