Cribriform ductal carcinoma in situ explained

A pathologist identifies the cribriform architecture in tissue taken during a biopsy or operation. The term describes how the cells are arranged; it does not by itself provide the whole risk assessment. The report may also describe nuclear grade, cell death within the ducts, the size or distribution of the area, hormone receptors and surgical margins.

Because the cells are in situ, they have not crossed the duct boundary into surrounding breast tissue. Careful sampling matters because a nearby invasive focus, if present, changes staging and treatment planning.

Arrange a breast-surgery consultation to match the pathology with the mammogram and any other imaging. A localised area may be managed with lumpectomy when clear margins are achievable. Mastectomy may enter the discussion when changes are widespread, appear in separate areas or cannot be cleared while preserving the breast.

After lumpectomy, the team may recommend radiotherapy based on the overall findings. If the DCIS is hormone-receptor positive, hormone therapy can be discussed to reduce the chance of another hormone-sensitive breast event. The balance of benefits and side effects is individual.

Contact a clinician for a new breast or armpit lump, bloody nipple discharge, nipple pulling inward, persistent breast-skin thickening or an unexplained change in breast shape. Following a procedure, urgent review is appropriate for fever, rapidly increasing swelling, spreading redness, pus-like drainage, shortness of breath or heavy bleeding. The urgency comes from the symptom itself, not from an assumption about its cause.

Request the exact grade, extent and receptor result, and ask whether the imaging and tissue findings agree. Find out how much tissue would need removal, how margin results could alter the plan, which side effects matter for each option and who will coordinate surveillance.
