Cramping and spotting around ovulation

Ovulation can irritate the surface around an ovary or cause a temporary hormone shift, producing a one-sided or central ache and a small amount of spotting. The timing can vary, particularly when cycles are irregular. Similar symptoms can occur with contraception changes, cervical irritation, infection, fibroids, or endometrial polyps. Chlamydia may cause pelvic pain, bleeding between periods, pain during sex, or discharge. The presence of a symptom does not identify its cause, so a new or changing pattern needs context.

Write down the cycle day, pain side and intensity, spotting colour and amount, and how long each symptom lasts. Include recent sex, contraception changes, missed pills, medicines, discharge, urinary symptoms, and whether pregnancy is possible. A clinician can use this history to decide whether you need a pregnancy test, infection check, examination, or imaging. Seek routine care when episodes repeat, interfere with normal activity, or do not settle as expected.

Get urgent medical help for sudden severe pain, pain focused on one side, heavy bleeding, fainting, severe dizziness, fever, or vomiting. If you could be pregnant, contact a clinician promptly for bleeding with pelvic pain, even when the bleeding seems light.
