Cramping and spotting mid-cycle

Some people notice a small amount of spotting and a short-lived ache around the middle of a cycle. Hormonal contraception, missed doses, emergency contraception, or pregnancy can shift the timing. Cervical irritation after sex may also cause light bleeding. Pelvic infection can bring pain, discharge, fever, or pain during sex. Fibroids and endometrial polyps can change bleeding patterns. A mid-cycle symptom pattern does not establish a diagnosis, and the pattern may be less informative when cycles are irregular.

Keep a cycle record showing the first day of each period, the day spotting appears, amount and colour, cramp location, sex, contraception, medicines, and other symptoms. Arrange a routine appointment if the pattern is new, lasts beyond a brief episode, returns across cycles, or affects daily activities. A clinician may discuss pregnancy testing, infection testing, contraception, and pelvic assessment. Use the treatment plan you have been given rather than changing hormones on your own.

Seek urgent care for severe or one-sided pain, heavy bleeding, fainting, marked dizziness, fever, or vomiting with pelvic pain. If pregnancy is possible, bleeding with significant pain needs prompt advice because the same symptoms can need different treatment in pregnancy.
