Cramping and spotting after sex

Cramping and spotting after sex can follow irritation or dryness, but repeated bleeding needs assessment to find the cause. Note whether the blood is on tissue or a pad, whether pain is superficial or deep, and whether this happens at a particular point in your cycle. Bleeding after sex is not a diagnosis by itself.

Friction, dryness or a sensitive cervix can cause light spotting and temporary cramps. Chlamydia and other infections may cause bleeding with discharge, pelvic pain or pain when urinating. Cervical dysplasia, cervical cancer, endometrial polyps or uterine fibroids can also present with abnormal bleeding; womb cancer is another condition a clinician may consider, especially when bleeding is new or unusual for you.

A clinician may ask about pregnancy possibility, contraception, menopause status, cycle timing, discharge and screening history. They may offer an examination, pregnancy test, swabs or further tests based on your symptoms.

Pause sex if it causes pain or bleeding, and use a pad to observe the amount rather than a tampon. Avoid fragranced products and consider a suitable lubricant if dryness may be involved. Book a clinician appointment if spotting recurs, follows sex regularly, or comes with pain or discharge. Bring a record of dates, amount, pain, contraception and any test results.

Seek urgent help for heavy bleeding, severe or one-sided pelvic pain, fainting, dizziness, fever, vomiting, shoulder-tip pain, or a positive pregnancy test with pain or bleeding. Prompt assessment is also needed for bleeding after menopause or bleeding that is steadily increasing.

Ask whether the cervix, vagina, uterus or an infection could explain the pattern, and whether you need pregnancy testing, swabs or an examination. Ask how to prepare for cervical screening or other investigations, which symptoms require urgent care, and when you should follow up.
