What the red rash may mean

Cradle cap, also called infantile seborrheic dermatitis, commonly causes greasy or flaky scales on the scalp. The skin underneath can look pink or red because it is inflamed. Mild redness may be more noticeable after washing, rubbing, sweating, or removing attached scales. Cradle cap itself is not caused by poor hygiene and is not something a baby catches from another person.

Redness outside the scalp, marked swelling, crusting, pus, bleeding, or a rash that seems very itchy can have another cause, such as irritation, eczema, or infection. A photograph and a description of when the redness started can help a clinician distinguish these possibilities.

What you can do at home

Wash the scalp gently with lukewarm water and a mild, fragrance-free baby cleanser, then pat it dry. If scales are firmly attached, soften them with a small amount of a plain emollient before bathing and loosen only what comes away easily with a soft brush. Do not pick, scratch, or scrape the skin, because this can create tiny breaks that become irritated or infected.

Avoid fragranced oils, essential oils, adult dandruff products, and medicated creams unless a healthcare professional has recommended them for your baby. Stop any product that makes the redness worse. If gentle care does not settle the rash, a clinician may discuss an appropriate emollient or, when indicated, a topical antifungal treatment.

When to seek medical care

Contact a healthcare professional promptly if the rash is rapidly spreading, hot, swollen, painful, blistered, bleeding, or producing pus or yellow crust. Seek urgent advice if your baby has a fever, seems unusually sleepy, feeds poorly, or appears generally unwell. Arrange a routine assessment when redness persists despite gentle care, returns repeatedly, or affects the face, body folds, or other areas.
