Cracked skin on the thumb that will not heal

Thumb fissures often reopen because the hand bends, grips, washes, and contacts soaps or cleaning products throughout the day. Pain can be sharp when the split stretches, even when the surrounding skin looks only mildly dry.

Frequent handwashing, sanitiser, wet work, friction, and cold air can strip surface oils. Contact dermatitis may cause persistent inflammation, while atopic dermatitis can produce dryness and itching. Psoriasis may create thicker, scaly skin that cracks under movement. A clinician should consider these patterns rather than assuming the split is simply dry skin.

Use lukewarm water and a gentle cleanser, then pat rather than rub. Apply a thick, fragrance-free emollient after washing and whenever the skin feels tight. Wear protective gloves for wet or chemical tasks, with clean cotton gloves underneath if sweating is a problem. Do not pull away attached skin or apply household adhesives to an unexplained wound; a pharmacist or clinician can advise on suitable dressings.

Book an appointment if the same crack repeatedly opens, limits thumb movement, or remains inflamed despite reducing irritants. Seek prompt care if redness spreads, the thumb becomes hot or swollen, drainage appears, pain escalates, or you develop fever. A persistent one-sided problem may need examination for contact triggers, dermatitis, or infection.
