Cough with tonsillitis: what to know

Tonsillitis commonly causes a sore throat, painful swallowing, swollen tonsils and sometimes fever or swollen glands. A cough is less typical of isolated bacterial throat infection, but it can occur when a viral illness affects the throat and airways together. Strep throat can cause marked throat symptoms without being the only possible explanation for a cough. Earache, worsening throat pain or a muffled voice can add useful clues.

Notice whether the cough is dry or productive, whether it began before the throat pain, and whether you have breathing difficulty. These details help a clinician separate throat irritation from infection lower in the airways.

Rest, drink fluids in small frequent amounts, and choose cool or warm drinks according to what feels soothing. Soft foods may be easier when swallowing hurts. Avoid smoke and other airway irritants. A pharmacist or clinician can advise on age-appropriate pain relief and cough measures, especially for children, pregnancy, or existing medical conditions. Do not start leftover antibiotics; antibiotic treatment is useful only when a clinician identifies a reason for it.

Seek urgent medical help for trouble breathing, inability to swallow saliva, drooling, rapidly increasing neck or throat swelling, severe dehydration, or a voice change with difficulty opening the mouth. Arrange a GP consultation when symptoms are worsening, swallowing remains difficult, the cough is persistent, or you are unsure whether the illness needs examination. A clinician may inspect the tonsils, listen to the chest and decide whether testing or treatment is appropriate.
