Mucus streaked with blood when coughing: what to know

A streak means blood is mixed along part of the mucus rather than filling it, but that description does not identify the source. Dry nasal passages, gum or throat bleeding, a recent respiratory infection and inflamed breathing tubes can all contribute. Bronchiectasis is another condition a clinician may consider when mucus and bleeding recur. Allergic rhinitis can cause nasal irritation and drainage into the throat. These possibilities are not diagnoses from the appearance of one sample. Mention how often it happens, whether the blood is bright or dark, and whether you have fever, chest pain or breathlessness.

Pause strenuous activity and sit upright. Record when the streak appeared, how often it has happened and whether it followed a bout of hard coughing. Drink fluids if comfortable, avoid smoking and vaping, and do not repeatedly clear your throat. Check for a nosebleed or bleeding gums without assuming they explain the episode. Tell your clinician about respiratory conditions and medicines that affect clotting. If you have a persistent cough or the streak returns, arrange review even when you otherwise feel well; the clinician may need to examine your nose, throat and chest.

Seek emergency care for breathlessness, chest pain, faintness, confusion, blue lips, or more than a small amount of blood. Get urgent advice if streaking is repeated or increasing, if you feel markedly weak, or if fever and the cough are worsening. A single small streak may still need prompt assessment when there is no clear explanation. Call ahead if you are unsure where to go, and describe the amount and associated symptoms clearly.
