What should you know about cough sounds like a dog barking?

The sound is usually produced in the upper airway rather than deep in the lungs. A viral infection can inflame the voice box and windpipe, particularly in young children, causing a rough, seal-like cough that may be worse at night. A common cold can also irritate the throat and voice box.

Other conditions can change the character of a cough. Acid reflux may irritate the throat, while allergic rhinitis can cause drainage that triggers repeated coughing. Asthma more often causes a dry cough, wheeze, chest tightness, or breathlessness, although cough sounds can vary. Acute bronchitis may follow a respiratory infection and can bring a persistent cough with mucus.

The sound alone cannot identify the cause. Age, fever, hoarseness, wheezing, mucus, exposure to illness, and how easily the person breathes are all useful clues.

Keep the person calm and upright, because crying and lying flat can make noisy breathing feel worse. Offer frequent sips of fluid if swallowing is comfortable. Resting the voice and avoiding cigarette smoke, incense, strong fragrances, and dusty air may reduce irritation.

For a child, do not use steam from boiling water because it can cause burns. Do not give cough or cold medicines to a child unless a clinician or pharmacist has advised that the specific product is suitable. Honey may soothe a cough for children older than one year, but it is not suitable for infants.

Arrange a clinical assessment if the cough is not settling, keeps returning, disrupts sleep, or occurs with fever, wheezing, vomiting, chest pain, or reduced drinking. A clinician may listen to the chest and upper airway and decide whether further testing or treatment is needed.

Seek emergency help immediately for struggling or very fast breathing, blue or grey lips, pauses in breathing, unusual sleepiness, inability to speak or cry normally, drooling with difficulty swallowing, or a child who cannot drink. A harsh, high-pitched noise when breathing in, especially while resting, can signal narrowing of the upper airway and needs urgent assessment.

Get prompt medical advice if the cough began suddenly after choking, eating, or playing with a small object. Sudden airway obstruction can occur without fever. Babies, people with significant asthma or other lung disease, and anyone whose symptoms are worsening should be assessed promptly.

You could ask: Does the cough sound come from the throat or chest? Could this be croup, asthma, reflux, or an infection? What signs would mean that we should seek emergency help? Which fluids, medicines, or inhalers are appropriate for this person? Should we return if the cough continues after the other symptoms improve?
