Cough and rash without fever: what should you know?

The absence of fever does not identify the cause. Allergic rhinitis or asthma can occur with cough and itchy skin or other allergy symptoms, while contact with a new product can cause a local rash. A viral illness may begin without fever or never cause one. Acid reflux can explain a cough but usually does not explain a new widespread rash. Consider when each symptom began, where the rash is, whether it itches or hurts, and any new food, medicine, detergent, plant, or animal exposure.

Take a photograph of the rash and keep a simple record of new products, foods, medicines, and exposures. Avoid suspected irritants and do not scratch. Keep the skin gently clean and use an unperfumed moisturiser if it is dry. A pharmacist can advise on suitable relief, but check before giving medicines to a child or combining products. Do not stop an essential prescribed medicine without medical advice unless emergency services instruct you.

Get emergency help for swelling of the lips, tongue, or throat, wheezing, severe breathing difficulty, faintness, or a rash that rapidly spreads with feeling unwell. Seek prompt review for purple or blistering skin, painful rash, eye involvement, persistent cough, or symptoms after starting a new medicine. A clinician should assess a rash that does not settle or keeps returning.
